They can be a real ems like we have had the inner nut start turning loose before the outer nut breaks loose occasionally. OK, I remember now.the "stud" in the pic is the "inner nut" which is really like a stud over top of a stud. Should be an "L" on the square of that nut. Yes, if outer nut is Left hand, inner will be the same.
